In consideration for my attendance and participation in this academy’s martial arts training, I, the student/ parent/ legal guardian, acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks. I further relieve Evolution Aquatics/ Nomad, MATI Clubs, M.A.T.I., its management, assigned staff, and fellow students, from any liability resulting in personal injury or loss of personal belongings. I also hereby state that the students named above are physically fit to take the prescribed course of instruction and do so of their own free will for an agreed upon fee. I grant and authorize MATI Clubs the right to take, edit, alter, copy, exhibit, publish distribute and make use of any and all pictures, video and/or audio taken of applicant to be used in and/or for any lawful promotional materials, included, but not limited to newsletters, flyers, posters, brochures, advertisements, websites, social networking sites and other print or digital communications without payment or any other consideration. I waive the right to inspect or approve the finished product wherein the applicant’s likeness appears, including written or electronic copy and waive any right to royalties or other compensation arising or related to the use of these images or recordings. I understand there is a NO REFUND POLICY on any monies I will pay to this academy or representative of said academy. A parent or legal guardian signature is required if the student is under the age of 18.

THIS IS OUR 4TH YEAR TO OFFER THIS CAMP DURING HOLIDAY BREAK!!! OPEN TO ALL STUDENTS FROM ALL LOCATIONS, BLACK BELT STUDENTS AND “NON-KARATE” STUDENTS FROM THE SURROUNDING COMMUNITIES.
LAST YEAR, IT SOLD OUT!!!

MATI CLUBS @ EVOLUTION CAMP DESCRIPTION:

We will provide/ includes a daily snack (no lunch), however, will provide lunch on Friday (pizza party). We will provide/ includes a water bottle, MATI Clubs karate t-shirt, break a board, karate medal on Friday, earn their next belt/ stripe (Black Belt students). We will also do 2 crafts and we will have visitors (fire/ police).

ELECTRONIC DEVICES WILL NOT BE ALLOWED TO BE USED DURING THE CAMP BY CAMPERS. PLEASE DO NOT BRING THEM.

This camp will be limited to the first 20 registered/ PAID students.
NEW STUDENTS WILL ALSO BE PROVIDED A WHITE BELT TO START THEIR KARATE JOURNEY! The student’s testing fee IS INCLUDED (a $50 value/ $25 value for Black Belt!) in the camp fee.

NOTE: Drop off for all campers if needed is as early as 8:30am and pick up is 12:30PM. If a student is picked up after the 15 minutes, a $10 late fee will be charged due to staff having to stay beyond their work scheduled end
time. Thank you for your understanding.
